    Prehospital critical care is associated with increased survival in adult trauma patients in Scotland

    Background Scotland has three prehospital critical care teams (PHCCTs) providing enhanced care support to a usually paramedic-delivered ambulance service. The effect of the PHCCTs on patient survival following trauma in Scotland is not currently known nationally. Methods National registry-based retrospective cohort study using 2011-2016 data from the Scottish Trauma Audit Group. 30-day mortality was compared between groups after multivariate analysis to account for confounding variables. Results Our data set comprised 17 157 patients, with a mean age of 54.7 years and 8206 (57.5%) of male gender. 2877 patients in the registry were excluded due to incomplete data on their level of prehospital care, leaving an eligible group of 14 280. 13 504 injured adults who received care from ambulance clinicians (paramedics or technicians) were compared with 776 whose care included input from a PHCCT. The median Injury Severity Score (ISS) across all eligible patients was 9; 3076 patients (21.5%) met the ISS>15 criterion for major trauma. Patients in the PHCCT cohort were statistically significantly (all p < 0.01) more likely to be male; be transported to a prospective Major Trauma Centre; have suffered major trauma; have suffered a severe head injury; be transported by air and be intubated prior to arrival in hospital. Following multivariate analysis, the OR for 30-day mortality for patients seen by a PHCCT was 0.56 (95% CI 0.36 to 0.86, p=0.01). Conclusion Prehospital care provided by a physician-led critical care team was associated with an increased chance of survival at 30 days when compared with care provided by ambulance clinicians

    New Ultraviolet Extinction Curves for Interstellar Dust in M31

    New low-resolution UV spectra of a sample of reddened OB stars in M31 were obtained with HST/STIS to study the wavelength dependence of interstellar extinction and the nature of the underlying dust grain populations. Extinction curves were constructed for four reddened sightlines in M31 paired with closely matching stellar atmosphere models. The new curves have a much higher S/N than previous studies. Direct measurements of N(H I) were made using the Lyα\alpha absorption lines enabling gas-to-dust ratios to be calculated. The sightlines have a range in galactocentric distance of 5 to 14 kpc and represent dust from regions of different metallicities and gas-to-dust ratios. The metallicities sampled range from Solar to 1.5 Solar. The measured curves show similarity to those seen in the Milky Way and the Large Magellanic Cloud. The Maximum Entropy Method was used to investigate the dust composition and size distribution for the sightlines observed in this program finding that the extinction curves can be produced with the available carbon and silicon abundances if the metallicity is super-Solar.Comment: ApJ, in press, 9 pages, 5 figure

    Associations between physical activity and stress levels in medical doctors working in New Zealand and Australia during initial COVID-19 restrictions

    In 2020, the world was gripped by the COVID-19 pandemic which put an unprecedented strain on health care workers. The aim of this study was to assess the effects of the Australian and New Zealand lockdowns on physical activity, depression, and anxiety in medical doctors. We hypothesized that during stressful times such as the COVID-19 pandemic lockdown, physical activity would have a positive effect on the mental health of medical doctors. Convenience sampling (using mass emailing via professional networks including medical associations) and snowball sampling were used during the early period of COVID-19 government mandated restrictions (25 March to 27 April 2020) in New Zealand and Australia. All registered medical doctors working in New Zealand and Australia were eligible to participate in the survey. The short survey collected information demographics, levels of physical activity and mental health using the International Physical Activity Questionnaire: Short Form and the Depression, Anxiety and Stress Scale-42. Of 469 participants who completed the survey, over 81% met the recommended physical activity levels (150 minutes of at least moderate-intensity physical activity/week). Physically inactive New Zealand and Australian medical doctors reported significantly higher depression (p = 0.006), anxiety (p = 0.008) and stress (p = 0.002) scores compared to their active counterparts. This study demonstrated that less physical activity was associated with higher anxiety and depression in medical doctors. A key recommendation from this study is to incorporate greater access to physical activity in healthcare settings for medical doctors

    Intraoperative electrocochleographic characteristics of auditory neuropathy spectrum disorder in cochlear implant subjects

    Auditory neuropathy spectrum disorder (ANSD) is characterized by an apparent discrepancy between measures of cochlear and neural function based on auditory brainstem response (ABR) testing. Clinical indicators of ANSD are a present cochlear microphonic (CM) with small or absent wave V. Many identified ANSD patients have speech impairment severe enough that cochlear implantation (CI) is indicated. To better understand the cochleae identified with ANSD that lead to a CI, we performed intraoperative round window electrocochleography (ECochG) to tone bursts in children (n = 167) and adults (n = 163). Magnitudes of the responses to tones of different frequencies were summed to measure the “total response” (ECochG-TR), a metric often dominated by hair cell activity, and auditory nerve activity was estimated visually from the compound action potential (CAP) and auditory nerve neurophonic (ANN) as a ranked “Nerve Score”. Subjects identified as ANSD (45 ears in children, 3 in adults) had higher values of ECochG-TR than adult and pediatric subjects also receiving CIs not identified as ANSD. However, nerve scores of the ANSD group were similar to the other cohorts, although dominated by the ANN to low frequencies more than in the non-ANSD groups. To high frequencies, the common morphology of ANSD cases was a large CM and summating potential, and small or absent CAP. Common morphologies in other groups were either only a CM, or a combination of CM and CAP. These results indicate that responses to high frequencies, derived primarily from hair cells, are the main source of the CM used to evaluate ANSD in the clinical setting. However, the clinical tests do not capture the wide range of neural activity seen to low frequency sounds

    Cryo-EM structure of the prothrombin-prothrombinase complex

    The intrinsic and extrinsic pathways of the coagulation cascade converge to a common step where the prothrombinase complex, comprising the enzyme factor Xa (fXa), the cofactor fVa, Ca2+ and phospholipids, activates the zymogen prothrombin to the protease thrombin. The reaction entails cleavage at 2 sites, R271 and R320, generating the intermediates prethrombin 2 and meizothrombin, respectively. The molecular basis of these interactions that are central to hemostasis remains elusive. We solved 2 cryogenic electron microscopy (cryo-EM) structures of the fVa-fXa complex, 1 free on nanodiscs at 5.3-Ă… resolution and the other bound to prothrombin at near atomic 4.1-Ă… resolution. In the prothrombin-fVa-fXa complex, the Gla domains of fXa and prothrombin align on a plane with the C1 and C2 domains of fVa for interaction with membranes. Prothrombin and fXa emerge from this plane in curved conformations that bring their protease domains in contact with each other against the A2 domain of fVa. The 672ESTVMATRKMHDRLEPEDEE691 segment of the A2 domain closes on the protease domain of fXa like a lid to fix orientation of the active site. The 696YDYQNRL702 segment binds to prothrombin and establishes the pathway of activation by sequestering R271 against D697 and directing R320 toward the active site of fXa. The cryo-EM structure provides a molecular view of prothrombin activation along the meizothrombin pathway and suggests a mechanism for cleavage at the alternative R271 site. The findings advance our basic knowledge of a key step of coagulation and bear broad relevance to other interactions in the blood

    Evidence of Embodied Social Competence During Conversation in High Functioning Children with Autism Spectrum Disorder

    Even high functioning children with Autism Spectrum Disorder (ASD) exhibit impairments that affect their ability to carry out and maintain effective social interactions in multiple contexts. One aspect of subtle nonverbal communication that might play a role in this impairment is the whole-body motor coordination that naturally arises between people during conversation. The current study aimed to measure the time-dependent, coordinated whole-body movements between children with ASD and a clinician during a conversational exchange using tools of nonlinear dynamics. Given the influence that subtle interpersonal coordination has on social interaction feelings, we expected there to be important associations between the dynamic motor movement measures introduced in the current study and the measures used traditionally to categorize ASD impairment (ADOS-2, joint attention and theory of mind). The study found that children with ASD coordinated their bodily movements with a clinician, that these movements were complex and that the complexity of the children’s movements matched that of the clinician’s movements. Importantly, the degree of this bodily coordination was related to higher social cognitive ability. This suggests children with ASD are embodying some degree of social competence during conversations. This study demonstrates the importance of further investigating the subtle but important bodily movement coordination that occurs during social interaction in children with ASD

    Determination of aortic valve area in valvular aortic stenosis by direct measurement using intracardiac echocardiography: A comparison with the gorlin and continuity equations

    AbstractObjectives. This study sought to 1) show that intracardiac echocardiography can allow direct measurement of the aortic valve area, and 2) compare the directly measured aortic valve area from intracardiac echocardiography with the calculated aortic valve area from the Gorlin and continuity equations.Background. Intracardiac echocardiography has been used in the descriptive evaluation of the aortic valve; however, direct measurement of the aortic valve area using this technique in a clinical setting has not been documented. Despite their theoretical and practical limitations, the Gorlin and continuity equations remain the current standard methods for determining the aortic valve orifice area.Methods. Seventeen patients underwent intracardiac echocardiography for direct measurement of the aortic valve area, including four patients studied both before and after valvuloplasty, for a total of 21 studies. Immediately after intracardiac echocardiography, hemodynamic data were obtained from transthoracic echocardiography and cardiac catheterization.Results. Adequate intracardiac echocardiographic images were obtained in 17 (81%) of 21 studies. The average aortic valve area (mean ± SD) determined by intracardiac echocardiography for the 13 studies in the Gorlin analysis group was 0.59 ± 0.18 cm2(range 0.37 to 1.01), and the average aortic valve area determined by the Gorlin equation was 0.62 ± 0.18 cm2(range 0.31 to 0.88). The average aortic valve area determined by intracardiac echocardiography for the 17 studies in the continuity analysis group was 0.66 ± 0.23 cm2(range 0.37 to 1.01), and that for the continuity equation was 0.62 ± 0.22 cm2(range 0.34 to 1.06). There was a significant correlation between the aortic valve area determined by intracardiac echocardiography and the aortic valve area calculated by the Gorlin (r = 0.78, p = 0.002) and continuity equations (r = 0.82, p < 0.0001).Conclusions. In the clinical setting, intracardiac echocardiography can directly measure the aortic valve area with an accuracy similar to the invasive and noninvasive methods currently used. This study demonstrates a new, quantitative use for intracardiac echocardiographic imaging with many potential clinical applications
